Abstract：Objective：To observe the clinical efficacy of She medicine Huoxue Jiangya Decoction in the treatment of H-type hypertension of phlegm blended with blood stasis type. Methods： A total of 80 patients with H-type hypertension of phlegm blended with blood stasis type who visited the Department of Cardiovascular Medicine at Lishui Hospital of Traditional Chinese Medicine from July 2022 to June 2024 were selected and randomly divided into a control group and a She medicine group （40 cases each） using the envelope method. The control group received conventional western antihypertensive drugs combined with folic acid for dual therapy，while the She medicine group was additionally treated with Huoxue Jiangya Decoction based on the control group. After four weeks of treatment，the clinical efficacy and incidence of adverse reactions were compared between the two groups，as well as the traditional Chinese medicine syndrome scores， levels of 24-hour ambulatory blood pressure and blood pressure variability， brachial-ankle pulse wave velocity and ankle-brachial index，and homocysteine levels before and after treatment. Results：After treatment， the total effective rate in the She medicine group was 87.50%（ 35/40），while that in the control group was 67.50%（ 27/ 40），with the difference between the two groups being significant （P＜0.05）. After treatment，the traditional Chinese medicine syndrome scores in both groups decreased compared with those before treatment （P＜0.05），and the score in the She medicine group was lower than that in the control group （P＜0.05）.Both groups showed a significant decrease in 24-hour systolic and diastolic blood pressure after treatment （P＜0.05），and the levels in the She medicine group were lower than those in the control group （P＜0.05）. The levels of day-night diastolic and systolic blood pressure variability were decreased in both groups after treatment （P＜0.05）， and the levels in the She medicine group were lower than those in the control group （P＜0.05）. Brachial-ankle pulse wave velocity was decreased in both groups after treatment （P＜0.05），and the level in the She medicine group was lower than that in the control group （P＜0.05）. The ankle-brachial index was increased in both groups after treatment （P＜0.05），and the level in the She medicine group was higher than that in the control group （P＜0.05）. Homocysteine levels were decreased in both groups after treatment （P＜0.05）， and the level in the She medicine group was significantly lower than that in the control group（ P＜0.05）. The incidence of adverse reactions was 17.50%（ 7/40） in the She medicine group and 22.50%（ 9/40） in the control group，with no significant difference between the two groups （P＞0.05）. Conclusion：On the basis of western medicine treatment， the addition of Huoxue Jiangya Decoction can improve clinical efficacy， reduce blood pressure variability and homocysteine levels，and improve brachial-ankle pulse wave velocity and ankle-brachial index in patients with H-type hypertension of phlegm blended with blood stasis type.
